To process your merchant application and determine your eligibility, please complete the form below.
- Business Profile: Enter your legal business name, contact details, and Tax ID/EIN.
- Processing History: We require a description of your goods/services and your estimated credit card revenue. Please upload your last 3 months of merchant statements so we can secure the best rates for your volume.
- Underwriting Authorization: As part of the approval process, we require consent to perform a standard credit check. Please review and check the authorization box at the bottom of the form.
